Conditions like Complex Regional Pain Syndrome (CRPS) or Reflex Sympathetic Dystrophy (RSD), Myofascial Pain, and many other presentations of chronic pain share a common feature: a disturbed tone of the sympathetic nervous system. This presentation describes the influence of the sympathetic nervous system on the musculoskeletal system, specifically as it relates to conditions treated by Physical and Occupational Therapists. The anatomy and physiology of the SNS is briefly reviewed. Factors that facilitate SNS tone are reviewed and guidelines to minimize SNS facilitation in patients are discussed. Two treatment approaches that have shown promise in managing syndromes characterized by high SNS tone are presented: Triggerpoint Therapy and Connective Tissue Massage.
