Course Description

TWO WAYS TO ATTEND:

Many of our pediatric and adult clients may have “reflux” as numerous diseases/disorders increase the potential risk of reflux. As treating therapists, our role is to provide education on lifestyle, positioning and diet modifications that may be beneficial based on evidence. But in order to do this, we need have more than a basic understanding of reflux and the impact it has on our patients – anatomy, lifestyle, and quality of life. But do they REALLY have reflux and if so, what type? Is it GERD, NERD, EE, BE, LPR or Refractory GERD? What is the agreed upon definition for these and how do they differ? What signs and symptoms do our patients have – esophageal vs. extraesophageal and how do we structure the patient interview to document these via the Reflux Symptom Index? We will discuss how reflux changes the anatomy based on the Los Angeles Classification System. In this course we build on your knowledge of reflux, define the types of reflux, how they differ, and review what the evidence says about the impact of lifestyle/diet modifications, medications including PPI’s and H2 blockers. The NEW guidelines for diagnosis clarify when and why specific tests such as the PPI test, endoscopy, manometry, or reflux monitoring should be done. Finally, these NEW guidelines identify specific surgical and endoscopic treatments that are available with evidence of benefit for patients who do not respond to pharmacological treatment.

Course Objectives

Upon successful completion of this course, participants will be able to:

  • Explain the differences between GERD, NERD, EE, BE, LPR and Refractory GERD
  • Summarize specific lifestyle and diet modifications that may be recommended and evidence behind these modifications
  • List surgical and endoscopic treatments available to individuals with reflux

Denise Dougherty, MA, CCC-SLP
Denise owns and operates a private practice in Indiana, PA, where she conducts therapy with both children and adults. She is a legal consultant/expert witness in wrongful death/negligence lawsuits involving dysphagia. Denise is a co-editor of Private Practice Essentials: A Practical Guide for Speech-Language Pathologists, published by ASHA Press. She has presented workshops and written numerous articles appearing in the ASHA Leader on the topic of private practice. Denise served as a consultant for The Arijah Children’s Foundation in Anguilla, BWI, providing evaluations and therapy programs for special needs children. Ms. Dougherty was an adjunct faculty member at York College of Pennsylvania and Wesley College in Dover, Delaware. She is a past President of the American Academy of Private Practice in Speech Pathology and Audiology (AAPPSPA), and received the Academy’s 2007 Honor Award and 2014 Award of Excellence for her achievements. Ms. Dougherty is a member of the Technical Experts Panel for Quality of Insights of Pennsylvania, working on initiating quality measures for CMS to improve effectiveness, efficiency, economy and quality of services delivered to Medicare beneficiaries. She received her bachelor’s degree in communication disorders from Marywood University in Scranton, Pennsylvania; a master’s degree in communication disorders from St. Louis University. Ms. Dougherty is certified in VitalStim® therapy and is a VitalStim® instructor, has completed additional training in orofacial myology, is a member of the American Speech, Language and Hearing Association and has received numerous ASHA ACE Awards.
