The swallow is a complex neuromuscular event. A symphony of nerve impulses travel back and forth between the cortex, the brainstem and the muscles in order to prepare the bolus and then guide it safely down towards the esophagus. Clinicians who treat swallowing disorders will seek to engage this system for therapeutic purposes; facilitate a muscle group, inhibit another; and enhance the feedback to retrain certain movement patterns, etc. The skillful application of such therapeutic interventions requires a thorough understanding of the anatomy involved in the swallow movements.
This course is designed to provide dysphagia professionals with the background necessary to participate in advanced courses that require detailed knowledge of swallowing anatomy. This information, will build the therapist's knowledge and skills for treatment of swallowing disorders and will provide the course participant with a stronger basis for a comprehensive examination of the dysphagic patient. The information provided in the course will also facilitate professional communication between the swallowing professional and other health care professionals involved in the treatment of dysphagia.

The world is more distant yet digitally connected than ever before. Technology has become our key to household management, social contact, entertainment, and adapting our occupations. The Pew Research Center indicates that U.S. Smart Phone users have increased from 35% in 2011 to 81% in 2019. Fifty percent of Americans possess a tablet and 75% own a computer or laptop. Immediate access to technology allows for a more efficient process but is often overwhelming. Knowledge, time, and experience helps to ease comfort with device, program, and application use. With more access, usability, and familiarity, use of technology in therapy correlates with functional utilization. As the Speech-Language Pathologists' (SLP) goal is to improve client outcomes and quality of life, apps and programs can encourage increased frequency of practice and innovate the care we provide.
This intermediate level presentation has organized the research, benefits, and limitations of technology utilization in various areas of our practice. Specific applications and programs are reviewed to increase the familiarity and knowledge of the evidence-based software options available to enhance SLP service delivery. The seminar serves as an instant resource to improve SLP clinician comfort and expertise in software management while providing useful tools and guidance to integrate technology as a treatment modality.

According to recent AAP publications, up to 50% of typically developing children and up to 80% of those children with developmental disabilities have feeding problems. With these staggering numbers, it is important to understand the underlying factors of infant and pediatric feeding and swallowing challenges. With the current trends in research highlighting the short term and long term effects of trauma, educating parents/caregivers on strategies to mitigate their infant/child's physiological and physical stress has never been more important. By implementing body work interventions, we aim to support the brain/body connection and help infants and children, birth-3 with any feeding and swallowing challenges they may have.

The four D's - dentition, dental care, dysphagia and diet are intertwined. This seminar will build on our basic knowledge and delve deeper into the role of dentition in maintaining our patient's health. What role does each tooth play in mastication? What disease processes wreak havoc on our patient's dental health? Dental care or oral hygiene takes a toll on our patient's health as well. Not only is dental care imperative for all individuals on our case load, but it is especially critical for individuals with specific diseases. We will discuss the impact of poor dental care on overall health. Individuals on our dysphagia caseloads have any combination of natural dentition, partial or full dentures or are edentulous. What role does dentition status and dysphagia play in diet selection? How does lack of dentition increase the patient's risk of aspiration? Rather than continuing to use the guidelines for the National Dysphagia Diet, the IDDSI should be implemented in our facilities. We will discuss how these two diets compare, and how to educate our staff to recognize and/or test foods to ensure the diet and liquid standards are met. New products that match the IDDSI levels will be introduced and reviewed. Many times, the patient and/or family reject the diet modification recommendations. Some facilities offer waivers, which are not recognized by the courts. Better options we implement are the Shared Decision-Making Algorithm and The Care Planning for Resident Choice.

Therapists face many challenges in the clinical arena today, with ever increasing caseloads and demands on time during the day. When learning to document in college, therapy students do not have the benefit of clinical experience, and thus do not truly know what they are learning at the time. Once in clinical practice they learn from therapist mentors and employers and piece together an idea of what is needed. There are times when a clinician is baffled that a patient receives a denial from the insurance company for therapy that seems so obviously needed. This course will help therapists to understand what third party payers are looking for and to gain a better understanding of how to present the big picture when documenting. Learn the true purpose of documentation, third-party-payer requirements, how best to communicate with your payers, how to paint a picture with your documentation, next steps after a denial, and how to keep your documentation simple.

In our role as Speech-language Pathologists we are to practice at the "top of our license", but often don't have the tools we need to get the job done. Most of us don't ask potential employers about the availability of therapy tools or a budget for therapy equipment. PRN staff often realize if they don't bring their equipment, there is nothing to work with. Budget constraints put us in a position of having to create or make our own tools. We can make some tools very inexpensively but there comes a time when purchasing specific equipment is necessary to document our outcomes. This seminar will look at tools we can create and utilize in our therapy room as well as those on the market to improve muscle function for an efficient safe swallow and provide documentation of our outcomes. Therapeutic interventions include swallowing and non-swallowing exercises but what are these and why do we use them? These exercises will be identified with literature discussing their efficacy, utilization and outcomes. In addition, new adaptive equipment for pediatrics and adults will be introduced that can assist with bolus control.

Childhood through young adult is a formidable time for cognitive development - but what happens when that is disrupted by an acute brain injury? Our assessments and treatment approaches need to be age appropriate for behavior, communication, and cognition. Functional cognitive skills allow us the ability to be functional and independent individuals -and that independence varies according to age. This course will review the neuroanatomy of the brain and theories of neuroplasticity. Participants will learn how to assess cognition and develop a treatment plan based on specific evaluations for acquired neurological injury in the medical model, and also address some of the challenges associated with this population.

Literature over the last 2 decades has shown an obvious link between motor function and cognition. Emerging data suggest therapies designed to improve specific aspects of cognitive function, may also improve gait, increase use of compensatory strategies, and reduce fall risk. Falls and fall-related injuries are a huge concern and costly burden to individuals, society, and healthcare systems. Despite the costly and physical consequences of falls, common interventions and 'fall prevention programs' have been shown to be inadequate or ineffective. This evidence shines a new light on the SLPs ability to target functional safety goals through cognitive therapy to change the outcomes of patient safety. The evidence-based techniques reviewed in this course are applicable to patients with fall concerns who mobilize in any way and/or participate in ADLs, including patients with Dementia, Parkinson's Disease, stroke, traumatic brain injuries, and other neurological diagnoses. This course focuses heavily on evidence-based cognitive therapy techniques to improve patient safety; along with education essentials for training of staff/family as a part of your plan of care. Discussion will include how to target these skills without duplicating services with PT/OT. Course material will cover goal writing for best practice standards and avoiding insurance denials. Specific patient examples and documentation from referral to discharge will be covered. Steps to initiate and/or increase the success of a current fall prevention program will be described. Barriers and challenges will also be reviewed.

Medical taping has a variety of applications within the context of musculoskeletal impairment and kinesiology taping techniques have gained the attention of rehabilitative providers. Therapeutic application provides re-education of the neuromuscular system, influence lymphatic drainage easing inflammatory fluid stasi while improving localized circulation and modulate pain. Diagnoses familiar to the Speech Language Pathologist in dysphonia and dysphagia benefit from its use. Taping strategies offer muscular inhibition as in craniomandibular hypertonus, muscular recruitment facilitating oral contro, hyolaryngeal movement/stability and scapular engagement while minimizing provocative postural compensations. This session seeks to educate the dysphagia clinician on the therapeutic benefit of taping to improve motor patterns and facilitate a client's functional independence.

So much of what we know about our world comes through our visual system. It affects reading, writing, posture, use of a communication board and so much more, yet we are taught so little about this system that has such a huge impact on the functional abilities of our children.
This engaging course will explain the anatomy and physiology of the visual system through evidence-based research and demonstrate the role vision plays in making academics more difficulty for some children. Therapists will learn to recognize the simple signs of undiagnosed vision-based learning impediments so they might better interpret the results of standardized testing. Through lab practice and case studies, attendees will gain confidence in assessing ocular motor and near vision focusing skills then design programs and use modifications to get the best possible outcomes. Attendees will then learn easy and fun techniques for improving these skills.

Therapists are challenged by patients with neurodegenerative diagnoses such as Parkinson's, Huntington's, Prion Disease or ALS. Providing appropriate therapies within our standard practice of care can make a difference in the outcomes. We must know the impact on cognition and swallowing, the impact of medications before we can decide what therapy tools/techniques will address their cognitive, swallowing and quality of life goals. This seminar will provide information on the diagnostic process, theories on development of the disease, symptoms that present in early as well as later stages. Medication protocols will be discussed as well as CAM or complimentary alternative medicine. Medications may improve quality of life for our patients but may also become ineffective. Numerous therapy techniques and tools available will be presented with rationale for their use. Recommendations for modified diets, liquids or even NPO may not be acceptable to the patient and/or family. As therapists, we need to know what options are available and educate/inform our patients and families to allow them to make decisions that work for them. Options will be addressed as well as concerns with waivers and need for appropriate, thorough documentation.

Neuromuscular Electrical Stimulation (NMES) is a relatively recent addition to the treatment toolkit of dysphagia clinicians. The modality was first introduced in a significant way in 2003 and has since seen a rapid adoption among Speech and Language Pathologists. It is estimated that over half of clinicians working with dysphagia have attended a continuing education course in the use of this modality.
Because of this growth and ongoing reports of positive patient outcomes in the peer-reviewed literature, the product and educational options have increased. Clinicians can now purchase different devices and they can choose to attend different continuing education courses.
It can be challenging for the clinician to discern what the best option would be. How do the devices compare? What are the therapeutic opportunities offered by each of them? What educational support is available? Are the educational offerings unbiased? Are there ethical ramifications to using one or other device?
This presentation will review most of the various devices and educational programs that are available to the dysphagia clinician today. The presentation will focus on known features as reported by the manufacturers and educational sponsors. Time will be left at the conclusion of the webinar to ask questions. The topic of ethical and legal ramifications related to the use of these devices will be briefly reviewed.

Feeding problems complicate the care of preterm and term infants in the NICU. Delayed oral feedings can lead to prolonged length of stay, need for G-tube, family stress and separation. These infants, typically demonstrate discoordinated suck-swallow-breath patterns, delayed oral motor skills and weak pharyngeal muscle movements - thus leading to penetration and aspiration, putting the infant at increased risk of pneumonia and future oral aversion. Traditional therapies have shown limited and slow progress in this fragile population. Infants unable to demonstrate adequate feeding abilities are often discharged with G-Tube and PEG placements. The use of NMES as part of feeding and swallowing therapy has grown exponentially over the last 20 years across all ages. Use of this modality in the NICU is also becoming more frequent. This short course will focus on the objectives of this therapy and how it can complement traditional therapies in the NICU when these neonates/infants struggle with dysphagia and discuss opportunities to help this fragile population perform their best.

It has long been established that the somatosensory system drives motor behaviors. This course will introduce "sensory point taping" (SPT) strategies for pediatric populations. This taping application method focuses on stimulation of the sensory system (Sensory), using the minimal effective dose (Point), with a gentle tape product (Kinesio Light Touch). The SPT method may be preferred for pediatric and geriatric populations with fragile skin. The instructor will discuss how elastic taping applications may be adapted to encourage motor control and safety as an adjunct to NMES.

Postextubation dysphagia or PED is not new; however, it has become more prevalent following long-term intubation of COVID-19 patients. Patients with COVID-19 have specific characteristics SLPS should be aware of. Impact of PED includes high prevalence of pneumonia, high mortality, risk of reintubation and poor outcomes. While there are no standard protocols for screening and diagnosis of PED, research has put forth various time frames for screening and assessment. This seminar will discuss the prevalence of PED, which patients may be more predisposed to PED including COVID-19 patients, clinical factors that increase potential for severe PED, recommendations for screening and evaluation as well as the impact of therapy to improve function and safety with oral intake.

This seminar will provide a road map to assist the professional who is interested in becoming a private practitioner. Private Practice is not for everyone and takes a lot of blood, sweat and tears. Before you begin the process, you need to examine the decisions necessary to build a strong foundation for a successful business. Many options are available for your practice - part time, full time, solo and partnerships and each has advantages and disadvantages to sort through. Many new businesses fail because owners failed to prepare themselves for the financial costs of start up and failed to formulate a strong business plan. Resources will be identified to assist you in writing a business plan and determining an appropriate business structure. Even though start up funds are limited, business consultants are important to your success. Referral sources, marketing and determining a fee structure will be addressed.

One arena in which nursing homes are utilizing therapists to meet the federal regulations is in restraint reduction. In moving towards a restraint free environment, the best approach utilizes a multidisciplinary Restraint Reduction Committee. Therapists play a unique role on the committee by assessing patients for balance and ambulation deficits, positioning issues, wound care and prevention and contracture management. Therapists are often asked to provide alternative positioning devices in lieu of restraints to promote safety. The committee implements trials of alternatives to restraints and a systematic process of restraint reduction is achieved.

Respiratory Muscle Strength Training is an important addition to our treatment program. Often our treatment programs do not include this modality. This course builds on our basic knowledge of respiration and its connection to speech and swallowing. However, respiration also ties into cardiovascular function and our immune system. These connections will be investigated. We will identify signs and symptoms occupational therapists, physical therapists and nurses should look for during interactions with their clients that would indicate the presence of respiratory muscle weakness. Various diseases and disorders contributing to respiratory muscle strength weakness including COVID-19 will be discussed as well as the use of respiratory muscle strength training in patient recovery and therapy programs. Inspiratory and Expiratory muscle strength training tools and their protocols will be discussed These will be presented with comparisons of similarities and differences. Studies relating to the efficacy of respiratory muscle strength training will be presented for numerous diseases, including COVid-19 throughout the course.

Surface electromyography (sEMG) is a valuable tool for rehabilitation of the patient with facial palsy. Audio and/or visual feedback can be used for neuromuscular retraining to facilitate recovery of function in patients with facial palsy. Delayed recovery is associated with higher incidence of synkinesis with more impairment of function. sEMG can also help these patients learn to inhibit or delink unwanted patterns.

Traumatic Brain Injury (TBI) affects the entire person and often the visual system. Patients who suffer a TBI require an interdisciplinary team approach to improve the overall function. The devastating effects on the brain may result in neurological changes to the visual system including double vision, blurred vision, headaches, and balance difficulties. Any of these can impede functional outcomes for all disciplines (i.e. gait training, activities of daily living, social skills, emotional appropriateness, language skills and perception). In this course the instructor will cover evidenced based research on neuroplasticity; causes of TBI; effects it can have on the person; assessment tools for education; and functional remediation exercises. This course will prepare OT/COTA's, PT/PTA's, ST's, educators, psychologists, nurses and other healthcare providers to better understand and treat the underlying visual factors that may be hindering progress. Participants will also gain a better understanding of when to refer to an optometrist/neuro-optometrist. This course will provide functional applications for you and your clients - from pediatrics to geriatrics.

Have you ever felt 'stuck' delivering therapy to kids with differently wired brains? Oftentimes, children with neurodivergent challenges (autistic, ADHD, etc.) require us to adapt and rethink traditional therapy methods. What if we had a framework for understanding how their brains think and sense the world? This introductory course for all therapy professionals is designed to inform, explore and celebrate the diverse range of neurological differences in the pediatric population. This course aims to foster a deeper understanding of neurodiversity, promote inclusivity, and provide valuable insights into the experiences of individuals with neurodivergent traits. Together we will learn how to support therapists' interactions with clients and create space for a mindset shift within our therapy practices. That feeling like nothing is working to progress towards goals is very real and disappointing to even the most experienced clinician. Let's ditch that together. This course will assist you in providing a new lens in which to view these children that ultimately will empower you and your thinking on how to build connection and improve therapy sessions.

Your patient complains of double vision, and dizziness. The chart says your patient is "legally blind". Your patient is having problems reading. These are common issues for adult patients of the rehab therapist, but the root causes of these issues are frequently misunderstood, so interventions are not effective. Therapists are presented the functional problems of visual dysfunction every day. Many therapists do not recognize these complaints as visual issues due to a lack of understanding of the changes that may occur in the adult visual system.
This course will help therapists recognize how naturally occurring changes in the visual system affect function. They will learn the questions to ask when your patient tells you they had cataract surgery, or they see double. Attendees will know the visual issues that can cause falls or reading problems and how to assess and improve the visual system so their patient can continue the activities they love. Attendees will learn to assess the adult visual system and develop activities to improve the most common visual complaints. They will even understand glasses better and be able to help their patient pick the eye care professional to best manage their visual complaints. This course will make you into a vision-aware therapist so you can recognize visual dysfunction better and provide interventions that get results where others have not.

Vision is our furthest reaching sense, telling us 75% of what we know about the world around us. Stroke, brain injury and concussion can disrupt the visual system, affecting not just our patient's knowledge of the world around them but also how they interact and move through that world.
This course will explain the most common visual consequences of stroke and brain injury from double vision, near vision focusing problems to visual cuts. Participants will learn to assess and document these deficits and learn modifications to improve performance and safety during treatment sessions in the clinic and at home. The participants will learn simple, inexpensive techniques to improve visual symptoms and patient outcomes. Physical, Occupational and Speech therapists will all benefit from a deeper understanding of the functional role of the visual system in their patient's recovery.

The VitalStim DINES Provider's Conference Keynote includes two wonderful presentations:
The first is John Michael and Vickie Night from "JM Strong 24" and Shannon Vogt, NMES Dysphagia Specialist
Shannon Vogt, John Michael's Speech Pathologist, presented her clinical strategies which brought John Michael from peg tube dependent to a successful outcome that experts said will never happen. John Michael was a senior in high school and having just signed his National Letter of Intent to play D1 lacrosse, life was good. Then one December morning it all changed. A massive brain stem stroke left John Michael locked-in, unable to speak or move anything but his eyes. With the worst possible prognosis, John Michael has defied the odds, including having the feeding tube removed following the dedication of his skilled dysphagia therapist.
The second presentation is Meredith Pauly and Chef Christopher Kearse discussing a fine dining experience for people with dysphagia and their families and the backstory of the Dinner with PALS (Persons with ALS) event, . Learn how they did it and get inspired to find a place at the table for people with swallowing disabilities in your own community.
